Applicants can choose to either apply online OR make an appointment to apply in person.

ALL applicants will be required to upload -OR- bring to your in-person appointment the following documents:
• Photo ID
• Birth Certificates for ALL children 12 and under –OR— proof of legal custody
• Proof of total household income (choose from the following)

1. • SNAP Notice of Action statement

1. • TAN-F Statement

1. • SSI or SSD Benefit Letter

1. • Paystubs for most current full month of employment

In addition, the following information will also be requested:
• 3 toy suggestions (must be under $75 per item)

— NO gaming consoles or console games, bikes, hover boards, tablet/phone, TV’s

• Favorite Color(s)
• Favorite Character(s)
• Registered child(ren) clothing and shoe sizes
